Location: a) Top of newly constructed fifth floor (new six story new building in progress) a) On or about 03/04/2022, employees stood and worked by the exposed edges on top of the newly constructed fifth floor without the use of fall protection to perform tasks such as, but not limited to, receiving materials, performing masonry work, and cutting materials .
