29 CFR 1910.147(c)(7)(iv): The employer had not certified that employee training had been accomplished and kept up to date: (a) Facility at 322 Water Street in Warren, RI: On or about March 4, 2022, the employer did not certify that employee lock out / tag out training had been accomplished for employees exposed to hazardous energy sources and amputation type injuries while cleaning or maintaining equipment, to include but not limited to, Ampco AL Series pumps, (Pump CJ#2).
