29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about March 3, 2022 at the jobsite, on the townhouse building, employees were exposed to fall hazards in that personal fall arrest systems included deficiencies such as but not limited to: a) Individual lifelines tied through the ridge vent opening around trusses were in contact with the edges of the metal truss connection plates; b) Roof anchors and rope grabs were not being used; c) One harness had a broken Dee ring support; d) One harness had corroded hardware; e) The harnesses of three employees were not adjusted to fit their body shape, f) The roof anchor of the lifelines were connected to the harness Dee rings;
