29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about February 24, 2022, and at times prior, the employer did not ensure that employees working at the host employer site were protected from the point of operation of the Recycletech Styrofoam extruder in the recycling area. This exposed Rite Staff employees to laceration and amputation hazards. (b) On or about February 24, 2022, and at times prior, the employer did not ensure that employees working at the host employer site were protected from the point of operation of the trash compactor in the recycling area in that the compactor was run with a disabled interlock and with the door open. This exposed Rite Staff employees to crushing hazards. (c) On or about February 24, 2022, and at times prior, the employer did not ensure that employees working at the host employer site were protected from the point of operation of the Harris horizontal baler in the recycling area. This exposed Rite Staff employees to crushing and amputation hazards.
