29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: Employees engaged in material handling activities at 10611 Gertrude St, La Vista, Nebraska, were exposed to electrocution and electrical shock hazards in that a circuit breaker panel had missing back plates that were covered with duct tape. The missing plates created openings that exposed employees to potential electrical hazards from the unprotected electrical bus.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: Employees engaged in material handling activities at the Magnum LTL facility located at 10611 Gertrude St, La Vista, NE were exposed to fire and electrical hazards associated with electrical circuit breaker panels blocked from access. Obstructed access to electrical panels and switches can prevent personnel from shutting down the source of power in the event of an accident.
