29 CFR  1926.451(b)(2):Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide.  On the east side of the construction site, employees engaged in cornice work on a residential home from single pole scaffold platform that was not 18 inches wide. The employees were exposed to potential fall hazard of approximately 12 feet to the ground below.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On the east side of the construction site, the employer and his employee worked from a pole scaffold without fall protection systems while installing cornice material on the exterior of a residential home. They were exposed to a fall hazards of approximately 12 feet to the ground below.

29 CFR  1926.452(a)(4): Diagonal bracing in both directions were not installed across the entire outside face of all double- and single-pole scaffolds.   On the east side of the construction site, employee(s) working from a pole scaffold without cross bracing installed were exposed to a fall hazard of approximately 12 feet to the ground below.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On the construction site, the employer did not develop or provide fall protection training to the employees engaged in cornice work to recognize and eliminate fall hazards when working over 10 feet from a single pole scaffold.
