Location: Jobsite located at 171 Everett Street Brighton MA.  On March 7, 2022, employees were exposed struck-by and crushing hazards while working near a granite stone foundation wall where the employer did not provide frequent and regular inspections of the work area.  March 7, 2022, An employee was injured when a granite stone foundation wall partially collapsed during hand digging operations.

On March 7, 2020, employees were exposed to struck-by and crushing hazards while working where the employer had not provided training on how to avoid a stone wall collapse during digging operations. On March 7, 2022, employees were exposed to struck-by and crushing hazards while working where an engineering survey was not performed by a competent person to determine the conditions of the residential structure in which employees were engaged in work operations.

On March 7, 2022, employees were exposed to electrical shock and gas explosion hazards while working in a structure under demolition where the electric and gas utilities had not been cut and capped outside the building.
