29 CFR  1910.28(b)(11)(ii):  Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: Stairway areas less than 44 inches (1.1 m) with two open sides did not contain one stair rail system with handrail on each open side; stairway areas greater than 88 inches (2.2 m), with two open sides, did not contain one stair rail system with handrail on each open side but did have one intermediate handrail located in the middle of the stair:  Location: 34 Loretto Street, Irvington, NJ 07111 - Main Office  / Loading Dock  a) Employees were exposed to fall from elevation hazards at the front office; stairways having more than 4 risers did not have the proper stair rail system on each open side in place on or about 3/7/22.  b) Employees were exposed to fall from elevation hazards at the loading dock; stairways having more than 4 risers did not have a stair rail system in place on or about 3/7/22.

One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  Location: 34 Loretto Street, Irvington, NJ 07111 - Manufacturing Area  a) Employees were exposed to amputation hazards while operating Line 1 thermoforming plastic plate machine with unguarded rotating rollers and gears of the internal trim station, on or about 3/7/22.

29 CFR  1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  Location: 34 Loretto Street, Irvington, NJ 07111 - Manufacturing Area  a) Employees were exposed to amputation hazards while conducting packing operations on Line 1 thermoforming machine that dispensed plastic plates with no guarding mechanisms installed near the internal trim station. An employee sustained a partial finger amputation near the point of operation. The incident occurred on or about 2/27/22.

Location: 34 Loretto Street, Irvington, NJ 07111 - Manufacturing Area  a) A reportable injury, which occurred on 2/27/22 and led to an employee sustaining a finger amputation, was not reported to the OSHA Parsippany Area Office within 24 hours. Violation was reported on or about 3/7/22.
