Parking closer than 8 feet from the center of railroad tracks is prohibited;  On or about May 31, 2022 the employer did not ensure that powered industrial vehicles were parked more than 8 feet from the center of railroad tracks.  The crane operator of the   439 Shipping crane was moving the rail mounted mule car towards the south to connect railcars. In this process, the mule made contact with the forklift that was on the west side of the tracks.

Items identified include but are not limited to , bridge brakes, bridge wheels, and main hoist.   c)  Between the dates of or about March 8, 2022 and June 30, 2022  the employer did not ensure that unsafe conditions revealed during crane inspections were corrected or repaired prior to the overhead crane being operated and utilized again. Overhead crane 439 had defects annotated in the daily inspections by operators which did not allow for the safe operation of the crane. Items identified include but are not limited to wire ropes, bridge brakes, cab windows, and trolleys.   d)  Between the dates of or about March 8, 2022 and June 30, 2022  the employer did not ensure that unsafe conditions revealed during crane inspections were corrected or repaired prior to the overhead crane being operated and utilized again. Overhead crane 441 had defects annotated in the daily inspections by operators which did not allow for the safe operation of the crane. Items identified include but are not limited to rails, bridge brakes, cab windows, and main hoists.
