(a) Location - 1600 Osgood Street, North Andover, MA: On or about 2/23/2022, Legacy Platform LLC, as a controlling employer did not ensure that frequent and regular inspections of the job site were performed by a competent person. As a result, subcontractor employees were exposed to Fall Hazards and Struck-by Hazards.
