29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone:  a. On or before March 7, 2022, the employer did not ensure that the Technician Procedure posted at Honey Roll Oven #2 clearly described the areas of the machine in which the jabber stick should not be used.  b. Production - On or about March 10, 2022, through March 20, 2022, the jabber stick provided by the employer did not fully enable operators to remove waffle cake patties from the rotating baking plate wheel on the Haas Honey Roll Machines Lines 1 to 4 without placing off/non-dominant hand in danger zone where there was the potential for exposure to machine hazards associated with rotating and moving parts.  c. Production - On or about May 2, 2022, the hand-held spray bottle provided to employees did not fully enable operators to season baking plates on the Haas Honey Roll Machine Lines 2 without placing their hands in the danger zone where there was the potential for exposure to machine hazards associated with rotating and moving parts.  d. Production - On or about May 20, 2022, the jabber stick provided to operators did not fully enable them to remove waffle cake cones from spindle molds of the carousel spinner, and to clean waffle cake cones and crumbs from the tabletop inside the spinner section on the Line 2 Haas Honey Roll Machine without placing their hands in the danger zone where there was the potential for exposure to machine hazards associated with rotating and moving parts.

29 CFR  1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for removal and transfer of lockout devices or tagout devices.   a. Production - On or about April 20, 2022, an authorized employee performed service and/or maintenance on the Line 2 Haas Honey Roll Machine No. 904762 (Kellogg No. 10525). The employer's Lockout/Tagout procedure did not include language clarifying that lockout/tagout devices should be removed in the numerical order set forth in the startup sequence column on page two of the procedure thereby exposing employees to mechanical, electrical, and/or pneumatic hazards when the machine is restarted after the completion of service and maintenance activities.  b. Production - On or about May 20, 2022, an authorized employee performed service and/or maintenance on the Line 3 Haas Honey Roll Machine No. 904766. The employer's Lockout Tagout procedure did not include language clarifying that lockout/tagout devices should be removed in the numerical order set forth in the startup sequence column on page two of the procedure thereby exposing employees to mechanical, electrical, and/or pneumatic hazards when the machine is restarted after the completion of service and maintenance activities.
