29 CFR 1926.1153(d)(2)(i):  The employer did not assess the exposure of each employee who was or may reasonably be expected to be exposed to respirable crystalline silica at or above the action level in accordance with either the performance option in paragraph (d)(2)(ii) or the scheduled monitoring option in paragraph (d)(2)(iii) of this section:  a.) 1745 Quentin Rd, Lebanon, PA 17042 - The employer did not conduct initial monitoring or provide objective data substantially similar to the operations to properly assess employee exposure to respirable crystalline silica for employees operating a walk-behind concrete saw, on or about March 9, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1:   a.) 1745 Quentin Rd, Lebanon, PA 17042 - The employer did not follow the Table 1 requirement to provide respiratory protection to properly protect employees exposed to respirable crystalline silica while operating a walk-behind concrete saw indoors, on or about March 9, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer did not provide the select or require the appropriate respirator to protect employees from exposure, on or about March 9, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
