29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about March 10, 2022 at the Spielman Highway liquor store project, the employer and employee were on the roof deck removing snow to start shingling the roof deck and no fall protection system was in use.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about March 10, 2022 at the Spielman Highway liquor store project, an extension ladder was in use to access the roof deck and the ladder did not extend 3 feet past the landing.
