29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) On or before March 9, 2022, and at times prior, employees were required to service and change blades on their saw machines without the benefit of a lockout/tagout program and were exposed to serious caught in and amputation hazards. The employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections.

29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:   a) On or before March 9, 2022, and at times prior, two employees were observed operating a forklift in the workplace. The forklift operators had not completed forklift training and evaluation prior to operating the forklift. This condition exposed the employees to serious struck-by hazards.

29 CFR  1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed:  a) On or before March 9, 2022, and at times prior, an employee was observed using a SpeedCut radial saw, Model #SSA17, at the wood shop area. The exposed blade of the saw was not provided with a guard cover. This condition exposed the employee to serious amputation and laceration hazards.

29 CFR  1910.213(h)(3): Radial saw(s) were not provided with an adjustable stop to prevent the forward travel of the blade beyond the position necessary to complete the cut in repetitive operations:  a) On or before March 9, 2022, and at times prior, an employee was observed using a SpeedCut radial saw, Model #SSA17 in the wood shop area. The saw had no adjustable stop preventing the forward travel of the blade which allowed the blade to travel beyond the work position. This condition exposed the employee to serious amputation and laceration hazards.
