29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with �1910.145 and removed from service until repaired in accordance with �1910.22(d), or replaced: a. On or about March 10, 2022, 1115 Ridgeland Parkway, Suite #104, Alpharetta, GA (Throughout Warehouse) - Employees were exposed to fall hazard. Employees were utilizing a 12' metal stairs ladder to access upper level storage racks to retrieve items from the upper level rack area. The metal ladder was observed having structural damages such as, severely bent horizontal and vertical structural support bar frame which is attached to the safety locking mechanism safety feature of the ladder and was not taken out of service.

On or about March 10, 2022, 1115 Ridgeland Parkway, Suite #104, Alpharetta, GA (Throughout Warehouse) - The designated, marked, and identified emergency exit door in the shop area was blocked with several large pallets filled with storage materials, making it difficult to access in the event of an emergency.
