29 CFR 1910.253(e)(5)(v): Hose showing leaks, burns, worn places, or other defects rendering it unfit for service were not repaired or replaced: (a) On or about 03/01/2022 - Employees were exposed to fire and burn hazards when performing repair work on roll off tarp arms when the hose containing oxygen was connected to welding equipment, cracked, and not maintained in a safe condition.
