On or about March 10, 2022, and at times prior thereto, at 16537 Portage Street, Justin, TX 76247, the employer did not train an employee to use an extension ladder. An employee installing metal flashings stepped onto the extension ladder's top rung, exposing the employee to fall hazards approximately six (6) feet above the ground.
