29 CFR 1910.22(a)(1):29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: a) In the electrical room - On or about March 11, 2022 , the employer exposed employees to illnesses while inspecting fire extinguishers and an eye wash station when walking through standing drainage water. b) In the breakroom - On or about March 11, 2022, the employer exposed employees to illnesses while using contaminated ice from an ice machine with a missing cover that had mold, dust and dirt build up inside. c) Throughout the workroom - On or about March 11, 2022, the employer exposed employees to illnesses while working in areas where dust and dirt accumulaiton on air vents had occurred.

29 CFR 1910.303(b)(7)(iv): Electrical equipment was not free of damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: a) At the dock: On or about March 11, 2022, the employer exposed employees to injury while using a lift platform to load and unload vehicles where a hand control pendent with an up and down control was damaged and missing a part of its cover.
