OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee exposing employees to struck-by and electrical hazards:  (a) On or about March 10, 2022 - 108 Von Braun Drive, Huntsville, AL, employees were exposed to struck-by and electrical hazards when daily and frequent inspections were not performed and deficiencies found during the annual crane inspection for the North American Industries Top Running Single Girder bridge such as, but not limited to, damaged safety latches, missing manual disconnects on control panels, missing covers on control panels, and capacity limits had not been corrected.

29 CFR  1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  (a) On or about March 10, 2022 - 108 Von Braun Drive, Huntsville, AL, employees were exposed to frostbite when employees did not know about protective gloves that were to be used when changing propane tanks.  Triton Stone Group of Huntsville, LLC dba Triton was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1541337, citation number 1, item number 2e and was affirmed as a final order on 1/18/2022, with respect to a workplace located at 108 Von Braun Drive, Huntsville, AL 35806.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) On or about March 10, 2022 - 108 Von Braun Drive, Huntsville, AL, employees were exposed to chemical hazards when temporary workers did not know where to find or did not have access to safety data sheets.  Propane was in use at the site as forklift fuel.  Triton Stone Group of Huntsville, LLC dba Triton was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(h)(1), which was contained in OSHA inspection number 1582920, citation number 2, item number 5  and was affirmed as a final order on 01/18/2022, with respect to a workplace located at 108 Von Braun Drive, Huntsville, AL 35806.
