29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) The Village, Lot 35, Madison, MS - On or about March 11, 2022 employees were exposed to a fall hazard while working on the edge of the roof without fall protection approximately 22 feet high.

29 CFR 1926.1053(b)(1):Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support: (a) The Village, Lot 35, Madison, MS - On or about March 11, 2022 employees were exposed to a fall hazard while climbing the 12 ft ladder to access the roof and the ladder that was not secured to the building. (b) The Village, Lot 35, Madison, MS - On or about March 11, 2022 employees were exposed to a fall hazard while climbing the 12 ft ladder to access the roof that did not extend 3 feet above the landing.
