29 CFR  1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  (a)  On or about March 09, 2022, where eight employees were shingling a new residential home at 2560 65th Avenue South, Fargo in North Dakota personal fall arrest systems were not inspected prior to each use for defects such as damaged lifelines,  the damaged lifelines were not removed from service.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about March 09, 2022, where eight employees were shingling a new residential home at 2560 65th Avenue South, Fargo in North Dakota where the employer did not require the proper use of ladders during roofing work where employees were allowed to continually use a ladder that did not extend three of more feet above the landing surface while installing roofing shingles.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) On or about March 09, 2022, were eight employees were shingling a new residential home at 2560 65th Avenue South, Fargo in North Dakota were permitted to operate pneumatic nail guns at approximately 120 psi and were exposed to eye and face hazards from flying particles while working on a residential roof without wearing impact resistant eye or face protection.   Aroca Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1)), which was contained in OSHA inspection number 1258358, citation number 1, item number 1 and was affirmed as a final order on November 30, 2017, with respect to a workplace located at 2008 Greyson Dr, Papillion, NE 68133.

Employees were exposed to fall hazards of up to 25 feet.   Aroca Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1)), which was contained in OSHA inspection number 1258358, citation number 1, item number 2 and was affirmed as a final order on November 30, 2017, with respect to a workplace located at 2008 Greyson Dr, Papillion, NE 68133.
