29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.   a-315 Rock Road, Glen Rock NJ 07452 (Library Roof): CSHO observed employees engaged in roofing activities and using a Werner portable aluminum extension ladder exposed to fall hazards of approximately 20ft to the concrete below. Condition noted on or about 03/14/2022.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a-315 Rock Road Glen Rock NJ 07452 (Library Roof): CSHO observed an employee engaged in roofing activities and positioned on an unprotected roofs edge in which a means of fall protection had not been utilized exposing the employee to fall hazards of approximately 20ft to the concrete below. Condition noted on or about 03/14/2022.
