In the marking department, on March 11, 2022, and times prior thereto, power tap was plugged into another power tap not required by UL Directory that contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle and not to be series-connected to other RPTs that is not required by the listing and labeling.

29 CFR  1910.305(g)(1)(iv): Flexible cords and cables were used for purposes prohibited by subparagraphs (a) through (e) of this paragraph:   In the mill department, on March 11, 2022, and times prior thereto, an extension cord was wrapped around a metal conduit, was used as permanent wiring to energize a computer and a table light,  exposing employees to electrical shock hazards.
