29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets:  a. At 3432 Lull Water Lane, in Orange Park, Florida 32073: On or about March 15, 2022, the employer exposed an employee to head injury hazards, in that a protective helmet was not utilized by an employee, while trimming and falling overhead tree limbs.

At 3432 Lull Water Lane, Orange Park, Florida 32073: On or about March 15,  2022, the employer exposed an employee to eye injury hazards cased by flying debris, in that eye protection was not utilized by an employee, while operating a chain saw to trim and fall overhead tree limbs.

At 3432 Lull Water Lane, Orange Park, Florida 32073: On or about March 15, 2022, the employer exposed an employee to fall hazards ranging from six to sixty-four foot, in that fall protection was not utilized by the employee, while performing tree trimming and falling operations while in an aerial lift.
