29 CFR 1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace.  On or about March 14, 2022 a medical evaluation of employees who were required to wear an N-95 respirator was not performed before employees were allowed to use the respirator. This condition exposed employees to health hazards.

29 CFR 1910.134(f)(2):Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator; whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.  On or about March 14, 2022, employees who were required to use N-95 respirators were not fit tested prior to their initial use of the respirator. This condition exposed employees to health hazards.

29 CFR  1910.134(c)(1)(viii):The written program did not contain provisions for training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance.  On or about March 14, 2022, employees who were required to use N-95 respirators were not trained on the proper use, limitations, and maintenance of the respirator. This condition exposed employees to health hazards.
