29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At a commercial worksite located at 701 E Hwy 22, Lebanon MO -- employees were dismantling a machine that had a section of the guardrails removed. Once the guardrails were removed, employees were exposed to fall hazards in excess of 16' above ground level and not tied off correctly with a personal fall arrest system (harness and lanyard). This was documented on or about 03/14/2022.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. At a commercial site located at 701 E Hwy 32, Lebanon MO -- Employees were exposed to fall hazards when dismantling a machine. While standing on the top of the machine, an employee was was using a plasma cutter to cut out sections so they could be lowered down. The employee was making 4 cuts, in a rectangular shape. After making 3 cuts -- two ends and one side, the employee stepped on it and fell through. The employee was not tied off to any anchor system causing the employee to fall approximately 16'8" to the ground. The employee broke his back and other bones and internal injuries. This was documented on or about 03/14/2022.

The holes varied in size throughout and were not covered to protect employees from tripping in or falling through. The holes were throughout the top of the machine, including where there were guardrails, and none of the holes were covered. This exposed employees to trip and fall hazards in excess of 16' above ground level. This was documented on or about 03/14/2022.
