29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about March 16, 2022, the employer at Avery Dennison Corporation at 8035 Lake Winds Drive at Oak Harbor, Ohio 43449 failed to develop and utilize procedures for the use of lockout and tagout devices on machine 1734 and its rollers when employees are required to enter the oven or place their hands on or within inches of the rollers inside the oven for cleaning the rollers, exposing employees to mechanical hazards.
