On or about March 15, 2022, employees were exposed to amputation and crushing injuries when performing activities such as changeovers and other activities on the CNC #198 WF spinner machine. The employer did not ensure energy control procedures were documented and utilized to properly control the electrical, hydraulic, and pneumatic energy before entering the WF spinner machine.
