29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about March 14, 2022, the employer did not ensure employees exposed to eye and face hazards caused by flying debris were provided with eye or face protection.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.  On or about March 14, 2022, the employer did not ensure employees engaged in residential construction activities and working at heights greater than six feet above the lower level were protected via personal fall protection means.

29 CFR  1926.1053(a)(2):Ladder rungs, cleats, and/or steps were not parallel, level, and uniformly spaced when the ladder was in position for use.  On or about March 14, 2022, the employer did not ensure the portable extension utilized by employees to access the upper working surface was positioned in a way that allowed the cleats to be flat and level on the ground while being used by employees.
