29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: (a) Great Divide Brewing Company at 2201 Arapahoe Street in Denver, Colorado: On or about March 16, 2022, and at times prior, the employer did not implement a permit space program to ensure safe employee entry into the mash tun. The mash tun met the definition of a permit space, and brewers regularly entered the space without removing safety hazards, such as the mechanical movement of the mixing rakes and hot water. This condition exposed employees to crushing and burn hazards.

The mash tun met the definition of a permit space, and brewers regularly entered the space without removing safety hazards, such as the mechanical movement of the mixing rakes and hot water. Employees were not required to isolate equipment or place lockout devices before performing the cleaning. This condition exposed employees to unexpected movement of the rakes inside the mash tun and hot water.
