29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: a) On or about December 20, 2021, Northbrook NH, LLC dba Deerfield Crossing at Northbrook did not ensure that employees were fit tested annually. Employees were required to use respiratory protection, including but not limited to, Honeywell N95 respirators when caring for residents that were positive for COVID-19, on quarantine or exhibiting signs of COVID-19 in the 1200 wing of the facility. No abatement certification or documentation is required for this item.
