29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter: a) Clara Maass Medical Center, 1 Clara Maass Dr., Belleville, NJ 07109 The employer provided and required employees including but not limited to Nurses and Housekeepers, to wear N-95 respirators. Nurses provided direct care to COVID-19 suspect and COVID-19 positive patients. Housekeepers performed duties including but no limited to cleaning the bathrooms and floors of rooms occupied by suspect and COVID-19 positive patients. The employees were not fit tested annually. Violation occurred on or about March 16, 2022.

29 CFR 1904.7(b)(4): When an injury or illness involved restricted work or job transfer but did not involve death or days away from work, the employer did not record the injury or illness on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and an entry of the number of restricted or transferred days in the restricted workdays column. Clara Maass Medical Center, 1 Clara Maass Dr., Belleville, NJ 07109 a) On or about 03/03/2022, the employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2022. On the OSHA 300 for 2022, case number 2022 00026 was entered for an employee who strained their back while working in 3NA. Column I was checked for this injury, indicating the employee remained at work, but received a job transfer or job restrictions due to the injury. The employer did not record in column L the number of days the employee was on job transfer or restriction. b) On or about 03/05/2022, the employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2022. On the OSHA 300 for 2022, case number 2022 00028 was entered for an employee who strained their shoulder while working in the Emergency Room. Column I was checked for this injury, indicating the employee remained at work, but received a job transfer or job restrictions due to the injury. The employer did not record in column L the number of days the employee was on job transfer or restriction. c) On or about 03/06/2022, the employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2022. On the OSHA 300 for 2022, case number 2022 00027 was entered for an employee who strained their ankle while working in Post Intensive Care. Column I was checked for this injury, indicating the employee remained at work, but received a job transfer or job restrictions due to the injury. The employer did not record in column L the number of days the employee was on job transfer or restriction.
