29 CFR  1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees:  Location: 106/107 Valleyview, Irvington, NY  a) On or about March 15th, 2022, employees were exposed to natural gas because pipes were not properly shored. Due to the trench collapse, the natural gas pipe feeding into the residential house broke at the connection point.

Location: 106/107 Valleyview, Irvington, NY  a) On or about March 15th, 2022, employees were exposed to the spoil-pile dirt collapse into the trench due to spoil pile not being at least two feet from the edge. Employees were exposed to cave-in hazard.
