29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  (a) West end interior, second level; The outer jacket of the power cord for a Dewalt brand Model DW303 reciprocating saw was damaged and open at the base of the strain relief boot, rendering the strain relief ineffective and causing pull to be transmitted to the terminal screws.

29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a) West end interior, ground level; A spliced 16 AWG flexible extension cord used to supply power to portable electric equipment had damage to the outer jacket in several areas, exposing inner wires.   (b) West end interior, second level; The outer jacket of the power cord for a Dewalt brand Model DW303 reciprocating saw was damaged and open at the base of the strain relief boot, exposing frayed inner wires.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Roof structure, various locations; Fall protection systems were not in place while employees performed framing work from top chords and/or webs of roof trusses at approximate heights ranging between 7-feet, 6 inches to 18-feet, 10 inches above lower levels.   (b) West end interior, second level; Employees were performing work near interior unguarded floor openings with edges approximately 10.5 feet above lower levels without the use of fall protection.

29 CFR  1926.1053(b)(13): The top or top step of a stepladder was used as a step:  (a) West end interior, intermediate stairway landing; The top step of a Gorilla brand 18-foot aluminum multi-position ladder configured as a stepladder was used as a step by employees while accessing and egressing the second level.   (b) East end interior, second level; The top cap of a Werner brand 6-foot fiberglass stepladder was used as a step by employees while accessing and egressing the roof structure.
