29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Building 4, 4th and Olive: On or about March 15, 2022, employees were using an extension ladder to access the roof of a two story building. Only one rung was positioned slightly above the eave. Note - Abatement certification is required for this item.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Building 4, 4th and Olive, Idaho Falls, ID 83401: On or about March 15, 2022, employees were exposed to falls between 20 - 25 feet while performing roofing activities on a 5:12 sloped roof without fall protection. Vision Investments dba Elite Roofing Systems, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1402058, citation number 1, item 1 and was affirmed as a final order on 09/27/2019, with respect to a workplace located at 790-796 Trails End, Idaho Falls, ID 83402. Note: Abatement certification and documentation are required for this item.
