29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: AVD is amended to read: On or before March 15, 2022, the employer did not instruct employees on safely entering an excavation with a trench box when employees were found accessing the inside of the trench box via a ladder and ladder hanger which was placed on top spreader bar. This condition exposed employees to fall hazards of approximately 11 feet. (a) On or before March 15, 2022, the creating and exposing employer did not ensure that employees from another company were protected from injury in, that an employee employed by another company accessed the inside of a trench box via a ladder and ladder hanger which was placed on a top spreader bar. This worker utilized the ladder hanger without the ladder attached. He sustained serious injuries when he fell approximately 11 feet inside the trench box when the ladder hanger displaced. This condition exposed employees employed by other companies to fall hazards of approximately 11 ft.
