29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 239 The Blvd, Carrick, PA 15210, On or about March 17th 2022, employees were not protected from fall hazards of approximately 25 feet while installing a new roof on a residential home that was measured at a  14/12 pitch. Employees were installing a new asphalt shingle roof and no fall protection was in use. i.e. harnesses, guard rails, etc.

29 CFR  1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  (a)  239 The Blvd, Carrick, PA 15210, On or about March 17th 2022, the employer did not conduct fall protection training for employees exposed to fall hazards of approximately 25 feet while installing a new roof on a residential home that was measured at a 14/12 pitch.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Job site 239, Pittsburgh, PA 15238: On or about March 17, 2022, during roofing activities, ladders were not extended at least 3 feet above the upper landing surface of the roof being accessed.
