29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Excavation located between Lots #32 to #37 Hastings Bridge Road, Lovejoy, GA 30250, Excavation area near Lot #37: On or about 03/17/2022, and at times prior, the employer exposed employees to struck-by hazards, in that an employee working inside an excavation was not provided with head protection from overhead hazards including the bucket of the Komatsu PC210 LC excavator.

29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   Excavation located between Lots #32 to #37 Hastings Bridge Road, Lovejoy, GA 30250, Excavation area near Lot #37: On or about 03/17/2022, and at times prior, the employer did not ensure that a safe means of egress was provided within 25-feet of lateral travel in the working area while engaged in sewer pipe installation activities.
