29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about March 14, 2022, an employee performing torch cutting activities received 2nd degree burns to the face and the employer did not ensure the employee was using appropriate face protection.

29 CFR 1926.352(i): Drums containers, or hollow structures which have contained toxic or flammable substances shall, before welding, cutting, or heating is undertaken on them, either be filled with water or thoroughly cleaned of such substances and ventilated and tested: On or about March 14, 2022, a employee received burns during torch cutting the bottom of a grain hopper tank which still contained combustible grain dust and the employer did not ensure the tank was thoroughly cleaned, ventilated, and tested prior to the application of heat.
