29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  (a)AB Custom Construction LLC, at 5841 South Corporate Place, Sioux Falls, SD 57108:  On or about and at times prior to March 18, 2022, the exposing employer did not ensure frequent and regular site inspections were being conducted by a competent person.  This condition exposed employees to various hazards such as falls, ladders, and forklifts.

This condition exposed employees to struck-by hazards associated with improper use of the forklift.
