29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems: Employees engaged in commercial roofing activities at the St. Stevens Episcopal Church, located at 422 West 2nd St, Grand Island, Nebraska, were exposed to fall hazards in that the employees were not utilizing a fall protection system. Employees were observed on a 12 in 12 pitch roof that was over 30 feet above the ground and they were not using any form of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed: Employees engaged in commercial roofing activities at the St. Stevens Episcopal Church, located at 422 West 2nd St, Grand Island, Nebraska, were exposed to fall hazards in that they used a ladder to access the roof and its sides rails did not extend at least 3 feet past the upper landing surface.
