29 CFR  1910.147(c)(4)(ii)(D):The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  (a) On or about 3/9/2021, and at times prior, at 110 Plugs Drive, Decatur, AL 35601, on Line 49, LOTO authorized employees working to thread/clear jam of sealing substrate on VisionPak were exposed to unguarded moving machine parts which could cause contusions, fractures, or disfigurement including amputation because the machine specific energy control procedures for the VisionPak did not clearly and specifically outline how to verify that stored pneumatic, water, and vacuum energy sources were deenergized to a zero energy state.

(a) On or about 3/9/2021, and at times prior, at 110 Plugs Drive, Decatur, AL 35601, on Line 49, employees working to thread/clear jam of sealing substrate on VisionPak employees were exposed to unguarded moving machine parts which could cause contusions, fractures, or disfigurement including amputation because when VisionPak components needed to move to thread the film, he sequence of actions described in 1910.147(f)(1)(i)-(v) was not followed.
