29 CFR  1910.37(a)(3):Exit route(s) were not kept free and unobstructed:  a) Administrative 2 (Bldg. 1233): On or about 1/19/2022, and at times prior, the employer exposed employees to fire and entrapment hazards, in that the exit route was obstructed when employees were required to go through the bathroom to reach the exit. The outside of the exit door had cables and wires  that dropped down across the exit discharge restricting use of the exit route while employees were performing administrative activities.   b) Clinic (Bldg. 1229): On or about 1/19/2022, and at times prior, the employer exposed employees to fire and entrapment hazards, in that the exit route was obstructed with a table and an extended partition across the route while  employees were observing and collecting urine samples for drug screenings.   c.) Drop-In Department (Bldg. 1223/1224): On or about 1/19/2022, and at times prior, the employer exposed employees to fire and entrapment hazards, in that the exit route was obstructed  with a steel barrel outside the exit discharge while employees were performing outreach support.  d.) Linkage Department (Bldg. 1225/1227): On or about 1/19/2022, and at times prior, the employer exposed employees to fire and entrapment hazards, in that the exit route was obstructed with metal piping while employees were performing administrative and field preparation activities.

29 CFR  1910.141(a)(5):Enclosed workplace(s) were not so constructed, equipped, and maintained to prevent the entrance or harborage of rodents, insects, or other vermin. A continuing and effective extermination program was not implemented.   (a) Facility: On or about 1/19/2022, and at times prior, the employer exposed employees to health hazards in that an effective extermination program was not established to prevent the entrance or harborage of rats when employees were required to conduct preparation for outreach activities.

29 CFR  1910.1030(f)(2)(i):Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I) or within 10 working days of initial assignment to employees who had occupational exposure::  (a) Facility; On or about 1/19/2022, and at times prior, the employer exposed employees to biological hazards, in that the hepatitis B vaccination or antibody testing for immunity was not provided to employees during their initial 10 days of occupational exposure while performing finger sticks and collecting blood samples.
