29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Flexline Line 4: On or about March 14, 2022, and at times prior, the employer exposed employees to caught-in and amputation hazards, in that periodic inspection of the written energy control procedures was not conducted at least annually to protect the authorized employees while performing service and or maintenance such as but not limited to clearing jams on Flexline Line4. The Specific LOTO procedures for Flexline Line 4 were last updated on April 3, 2019.
