29 CFR  1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  a) On or about March 21, 2022, employees at Plastipak Packaging, Inc. were exposed to caught-in, struck-by and crushed-by hazards, in that they were not trained in the procedure to conduct hazardous energy control of the Line 38- SWF Machinery Downstream "box making machine" while reaching in to clear jammed cardboard product.   b) On or about March 21, 2022, employees at Plastipak Packaging, Inc. were exposed to caught-in, struck-by and crushed-by hazards, in that they were not trained in the procedure to conduct hazardous energy control of the Line 42- SWF Machinery Downstream "box making machine" while reaching in to clear jammed cardboard product.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) On or about March 21, 2022 at Plastipak Packaging, Inc., employees were exposed to caught-in, struck-by, and crushed-by hazards in that the employer did not ensure each employee affixed a lockout device(s) to energy isolating devices of the Line 38 - SWF Machinery Downstream "box making machine" in a sequential manner which included the application of lockout devices to control the mechanical, electrical and pneumatic energy sources while reaching in to clear jammed cardboard product.   b) On or about March 21, 2022 at Plastipak Packaging, Inc., employees were exposed to caught-in, struck-by, and crushed-by hazards in that the employer did not ensure each employee affixed a lockout device(s) to energy isolating devices of the Line 42 - SWF Machinery Downstream "box making machine" in a sequential manner which included the application of lockout devices to control the mechanical, electrical and pneumatic energy sources while reaching in to clear jammed cardboard product.

29 CFR  1910.147(c)(5)(ii)(D):Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s):  a)  On or about March 21, 2022, employees at Plastipak Packaging, Inc. were exposed to caught-in, struck-by and crushed-by hazards, in that the SWF Machinery Downstream "box making machines" were not locked out and tagged out with the identity of the employee placing the lockout and/or tagout device, prior to employees placing any part of his or her body where an caught-in, struck-by and crushed-by hazard exists while performing maintenance and service activities on the "box making machines."  SWF Machinery Downstream "box making machine" Unit #1287 was locked out with a silver colored Masterlock lockout device, which did not indicate who placed the lock, or the reason for the lockout/tagout.  b)  On or about March 21, 2022, employees at Plastipak Packaging, Inc. were exposed to caught-in, struck-by and crushed-by hazards, in that the SWF Machinery Downstream "box making machines" were not locked out and tagged out with the identity of the employee placing the lockout and/or tagout device, prior to employees placing any part of his or her body where an caught-in, struck-by and crushed-by hazard exists while performing maintenance and service activities on the "box making machines."  SWF Machinery Downstream "box making machine" Unit #13241 was locked out with a red colored lockout device attached to a group LOTO hasp, which did not indicate who placed the lock, or the reason for the lockout/tagout.
