Vernon located at 13 Avalon Road, Mount Vernon, Ohio 43050: On or about February 27, 2022, the employer did not provide an annual respirator fit test for employees who were required to wear a N95 respirator including, but not limited to, Makrite Industries Inc. SEKURA-N95 respirator to protect employees from exposure to SARS-CoV-2 (the virus that causes COVID-19 disease).

Vernon located at 13 Avalon Road, Mount Vernon, Ohio 43050: On or about February 27, 2022, the employer did not provide annual respiratory protection retraining to all employees required to wear respirators. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents.
