OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees when emergency devices were inadequate. On the production line, employees worked at a conveyor when the emergency stop pull cords were not working while the conveyor was running.

29 CFR 1910.28(b)(3)(ii):(ii) Each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. Behind the production line, a long floor hole (water channel) was not covered to prevent employees from stepping/tripping into.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. In the production line area, employees walked under the conveyor and there were no guards in place to prevent employees from being caught in the moving parts.
