On or about March 22, 2022, workers installed shingles on a residential home under construction without frequent and periodic inspections by a person competent, exposing the workers to construction hazards including but not limited to; fall hazards up to approximately 14 feet high.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b). On or about March 22, 2022, workers installed shingles to the roof of residential homes under construction up to approximately 14 feet high without wearing fall protection, exposing the workers to a fall hazard.
