29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On or about March 22, 2022, and at times prior thereto, four employees installed shingles on residential homes under construction without frequent and periodic inspections by a person competent in recognizing fall hazards, exposing the employees to construction hazards such as but not limited to; fall hazards up to approximately 15 feet high.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  On or about March 22, 2022, and at times prior thereto, four employees were using pneumatic nail guns to install shingles to the roof of new homes under construction without wearing appropriate eye protection, exposing the employees to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  On or about March 22, 2022 and at times prior thereto, five employees were installing paper and shingles on the roof of the residential home under construction without the employer providing fall protection, exposing employees to a fall of approximately 15 feet.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  On or about March 22, 2022 and at times prior thereto, five employees were installing paper and shingles on the roof of the residential home under construction without the employer providing fall protection training, exposing employees to a fall of approximately 15 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails was not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.  On or about March 22, 2022, five employees accessed the roof of a one-story house by using a portable ladder that was not extended at least 3 feet above the upper landing surface, exposing the employees to a fall hazard of approximately 9 feet to the ground below.
