29 CFR  1910.133(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about March 22, 2022, and at times prior, at jobsite located at 4225 W. 20th Avenue Hialeah, FL 33012, the employer did not ensure employees who were exposed to hazardous chemicals such as, but not limited to Clorox Germicidal Bleach, a corrosive chemical, and BETCO Botanical Disinfectant Solution AF79, also corrosive, used appropriate eye or face protection while performing janitorial services.

29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about March 22, 2022, and at times prior, at jobsite located at 4225 W. 20th Avenue Hialeah, FL 33012; the employer did not ensure employees who were exposed to hazardous chemicals such as, but not limited to Clorox Germicidal Bleach (Sodium Hypochlorite) a corrosive chemical, and BETCO Botanical Disinfectant Solution AF79, a corrosive chemical, were provided with suitable facilities for quick drenching or flushing of the eyes and body within the work area when performing janitorial services.

29 CFR  1910.134(e)(6)(i):The employer did not obtain from the physician or other licensed health care professional (PLHCP)  a written recommendation regarding the employee's ability to use the respirator: contained the following information: Any limitations on respirator use related to the medical condition of the employee, or relating to the workplace conditions in which the respirator will be used, including whether or not the employee is medically able to use the respirator; (B) The need, if any, for follow-up medical evaluations; and (C) A statement that the PLHCP has provided the employee with a copy of the PLHCP's written recommendation.  On or about March 22, 2022, and at times prior, at jobsite located at 4225 W. 20th Avenue Hialeah, FL 33012; employees were required to wear 3M N95 filtering facepiece respirators and the employer did not obtain a written recommendation from a physician or other licensed health care professional regarding the employee's ability to use a respirator.
