On or about March 22, 2022, and at times prior, at jobsite located at 4225 W. 20th Avenue Hialeah, FL 33012; employees were required to wear 3M N95 filtering facepiece respirators and the employer did not obtain a written recommendation from a physician or other licensed health care professional regarding the employee's ability to use a respirator.
